I honestly can't think of any way that could connect her and Shinichi. At this point I'm hoping/wondering if the translation about her "knowing Shinichi/Conan was a Holmes fan for a long time" was somehow wrong/not complete. If it's not, that just doesn't seem to add up. In the conclusion of her entry case, she seemed surprised at the name Kudo Shinichi, like she didn't know who he was. Even in the file following that, the beginning of the bomber case, she says "Kudo?" when her classmates mention him.
I guess the only thing to do is wait until the new year and hope for some answers :V
In Sera's defense, Sera's personality is awfully friendly. It didn't take long after meeting Ran for her to apparently become best buds. She was literally hanging on Ran and Sonoko in her second appearance. (771) People commented about her general forwardness back then.
I remember at least one person theorized she might be lesbian or something like that. Ask Abs. : P
I'm waiting for an official translation before I try to judge Sera's line fully, but it sure sounds like she has done the research and figured out Conan is Shinichi. Spending the time needed to figure that out might produce some feeling of attachment and concern for the person, especially if you are prone to it in the first place.
I just can't wait until she actually meets Akai. I don't know whether she's going to Jeet kun do him or give him a full body hug until he turns blue.
Re: how Sera knows Shinichi, remember Akai used to keep this picture in his truck? He should have taken it out before the meeting with Rena to avoid the police or Org finding it later. Where did he put it? Probably with his personal stuff, because that's faster and giving it to the FBI all of a sudden might make them ask why he has given it back. What happens to a dead guy's personal stuff (when they work for the US)? The stuff get sent back home to the fmily. If Sera found the picture, she would definitely try to figure out who all those people are. Kudo Yukiko will likely lead to Kudo Shinichi and Conan is famous for chasing Kid. (Haibara even conveniently mentioned that this arc in a Sera case) Haibara isn't famous for anything, but Haibara is the biggest in the picture so much of Sera's investigation will likely be focused on her, which explains Sera's interest in Agasa's glasses nicely.

The stage case would've been credited to Yukiko or Radish Redwood. Shin'ichi wasn't given the credit for that. The only case related to America he could've gotten credit for was the one on the plane to New York. Akai while in New York chasing Vermouth/the serial killer doesn't know about Shin'ichi at all at this point (he only bumps into Ran), so I don't think it's possible for him to have mentioned Shin'ichi to Sera. It certainly doesn't seem like Shin'ichi and Sera bumped into each other at some point either.
Now it is possible that wanting to keep up to date on events in Japan that while in America Sera kept reading Japanese newspapers or such (maybe also in hopes of figuring out what her big brother was up to). So, technically she could have learned about Shin'ichi through news reports while she was located in America.
