She was supposed to kill Conan-Shinichi, but she won't because he was her silver bullet to destroy the BO.
That's why I thought that maybe she was a spy in the BO herself.

Ai's assumption was not definitely based on appearance, and should be on the deduction skills Okiya showed during that time. Thus, she must know a person having that kind of skill (besides Conan, ofc). Though the last statement (Haibara feels a stormy atmosphere for the men in black.So it's Subaru after all!?) really confuses me. The first sentence says that Haibara most probably assumes that it's a BO, and the second sounds to me like "So Subaru is Bourbon after all!?". But Bourbon is new, so Ai (most likely) haven't met this guy...

That would be a reason for Ai's first defensive reaction towards him - she 'felt' the black organization somehow around him, but not anymore. And I am pretty sure if Akemi had a crush on a man she would tell the only person she trusted: Ai. That doesn't mean Ai needs to know how he looks or that she met him before. For those little detectice freaks it is already enough to know, that there was a man ... .

However that was discussed already 100.000times last time I checked

Well at least I know Akai's former codename was "Rye"; "Moroboshi Dai" before that

*Okiya drinking Bourbon - just to make readers speculate I guess...
But he can't definitely rejoin the BO unless he masks as a different entity - Okiya maybe, as many assume. And Akemi might not tell Ai about Akai; Akemi didn't even know Akai's true name (she only knows his fake name - Dai in that case), so Ai knowing Akai is a faint (yet possible) possibility. And too bad, but Ai ain't detective freak like Conan

But I don't remove the possibility of your assumption. Ai might even turn out to be wrong. But at least, Conan's face in the past chapters is telling us something about Okiya's identity; and sorry to Ai fans, but I believe that Conan is more reliable
