Conan seems to be under the impression Shiho knows a lot about Akai. According to Conan's deduction in the hospital, Akai got close to Akemi to get close to Shiho who then introduced him to the Organization. So Shiho would most likely know who Akai is and Akai himself is aware of this, which is why he tries to stay out of sight when dealing with Shiho.
What I want from that scene:
Shiho slapping Conan, Shiho telling Conan that he had no right to keep something so personal from her, and running out of Agasa's house, into the Beika shopping district.
Of course Conan gives chance like a mad man(again) only to get caught up in a murder in the district, so he has to solve the case with a timer on his head, a timer that has him thinking each second is a second Shiho disappears forever*dramatic music*
...Though of course she comes back at the end of the file and turns out she was never running away(again). She just ran next door, took Shinichi's credit card, and then ran to the shopping district to buy herself three very expensive bags to get even.....
...we still don't know what's his relation with her...
