Jim and John are in space either in a spaceship or a space station. It was hit by a meteor and now has an air leak. John went out of the ship/station to try and fix the air leak, but something went wrong and his life line got cut. Jim is left on the leaking air ship/station but has no idea how to fix the leak. If he goes out side he has no one who can get him back and they used the only life line they had. Either way because of John failure, Jim is now doomed
Spoiler:
This would also work underwater only this time instead of dying by lack of air, they would drown and the accident to the sub or underwater lab could be an earthquake that cracked the dome

Update
Spoiler:
Since Jim even though he has no good way of fixing the hole, he decides to go for broke. He goes out side and takes a look at the hole. It is bigger then He thought it was. Realising he can not fix it he tries to get back to the airlock, but his motions instead move him father away. After an hour, he gives up and removes his helment and dies
