I don't think Ai is going to take the permanent antidote in the end. She has nothing to live for as an adult. Even if she loves Shinichi, Shinichi loves Ran and Ran loves Shinichi in return so there is no happiness to be gained by trying to insert herself into their relationship. Her sister and parents are dead and she will be wanted for her crimes as an adult. All her relationships with people have been created in her Ai form, and aging would put a damper on some of them, especially the DB ones which she treasures. I think she will choose to start over again as a child.
