I know if examined, my theory would probably be full of holes. However, I refuse to believe that "Akai = Okiya". Because of how easily Chekov exposed this. I wish Aoyoma tried harder in building this Araide-like twist.
But, whatever, it's fun making things up.

You just raised the main problem here. Gosho is trying to build this Araide-like twist. The question is WHY? Why should he build a similar twist to one which he has already used? It's unoriginal.

Yeah, the whole Akai=Okiya thing makes a whole lot of sense after reading all the theories and whatnot. And, I think, Akai's presence has triggered Haibara's senses previously as well, which would possibly explain her initial feeling when Okiya was first introduced.

Haibara's BO senses went off.

I think that time it was Vermouth. But something did happen in Chapter 383 (or episode 311) with Akai.

After Conan's confrontation with the BO and Haibara found him, Akai walked passed the two and Haibara had a scared look and looked back to see who it was.
