Before I start to tell you about my theory, let's remember about how Vermouth become Araide.
She wears a mask which it's really identic with Araide's original face.
No one can find the difference.

Now my theory, FBI create a fake character names "Shuuichi Akai".
His (Akai's) face is just a mask just like fake Araide's mask created by Vermouth.
Since Akai joined BO as "Rye" until his "death" at Raiha Falls, his face is totally a mask.
Then, who run Akai's character?
Of course a skillful agent who has the sniping ability and intelligence, just like Akai's.
But he's not Akai because Akai is just a fake character to manipulate BO.
This agent wears a mask of Akai this long, act like a person names Shuuichi Akai.
If this is true, then there's no problem about Akai's death in Raiha Falls, is he really dead or no.
Because it can be another people who wears the mask and act as Akai.
Of course that's not that skillfull agent who always run Akai's character.

Now, who's that skillfull agent who run Akai's character this long?
My suspect is... Subaru Okiya.

Good theory except there were some signs that tipped off extremely sharp fans, like the extensively discussed type of glasses worn by Araide and by Vermouth, because her nose being different, an other type of rim had to be used

Wow, if Aoyama did that, it would definitely be another epic plot twist in the series.
