from what i could understand without the subs..

when shinichi went into house library.. ran was there crying.. so she told him 'don't come conan-kun, i don't want you to see me crying.. so shinichi was very sad and he took off his glasses and told Ran that he is shinchi.. especially after hearing sonoko saying araide proposed to her.. i think he was very anxious.. but ran put his glasses back and didn't believe that he was shinichi.. thinking that conan said that to make her feel less sad.. so shinichi asked what she's going to do about araide.. and ran said she's going to reject him.. i think after this she said she will wait for shinichi.. this is where i couldn't understand well.. 

sorry if i sound boring or weird.. i'm quite bad at describing scenes..

it made me cry when ...

SHinichi said "(the sad song starts )Because ... It is ME.. because i am shinichi ... so ...(cant understand the next) and then the camera effects and when ran didn't believe him!!

and i love the blue light effects!! and their drawing and animation in this Ova

I still think Shin'ichi's request for her to wait for him has put Ran in a situation where she can't be happy with whatever choice she makes. If she moves on, and he shows up again, she's feels unfaithful and regrets never knowing what might have been. If she waits and he shows up much later or never, she's foolish for missing out on chances to be happy with someone else. I have no idea why folks want to hold it against Ran for being in that situation, when Shin'ichi put her there (yeah, yeah... she should get out of Haibara's way... when she doesn't even know she's "in the way" and Shin'ichi is the one who asked her to wait... pfff...)

Kinda interesting that Shin'ichi nightmare is apparently that Ran did move on rather than lead a sadder life giving up romance* to wait for him... Is he really that much of an as-- uh, egotist? Meh, I guess an OVA can't be taken all that seriously for characterization.

* No, I don't think all women need monogamous lifelong faithful romantic relationships to have a content life. I bet Aoyama's Ran would think she's missed out if she doesn't at least have a bit of that romance part though. Not sure about Aoyama's Haibara...

Shinchi told her to wait, but whether she chooses to do so is on her. Shinchi didn't put her in any situation...she put herself there. She needs to get over that arrogant egotist (Like that word you used  ) And get with someone who will give her the time.If she thinks waiting for him will change anything, she'll be waiting for him for the rest of her life as he is a detective and still won't even have the time. Otherwise she would be what you call settling for less when she could actually be happy with someone who cares about her. If an OVA is there its meant for something....As she gets older she will grow up and she could let Shinchi know that a woman can't wait on dreams when the whole world is waiting her. That would shut the egoist down, wouldn't it? High school girls do stupid things because they use their hearts instead of their minds. When she gets grown she'll be using her mind.  Especially if she's waiting that long.....

A short summary from my crappy memory:

Spoiler:

Conan has a fever. Ai calls him because she's made a new antidote. Conan runs over to Ai and Agasa to test it (I think Ai said it would only work for some hours). Conan takes the antidote. When he gets out of the bathroom after changing clothes, everything seemes to have changed. The city is different, the small tree in his garden is huge, etc). He goes to the school to meet Ran, but there he only meets new teachers and the classes are different. Then he meets Ayumi, Mitsuhiko and Genta, and later Ai. All 17 years old. Conan takes Ai out to talk with her and I didn't understand a word ;_; Ai didn't seem surprised, so I don't know if he was talking about any "time traveling" stuff or anything.
Conan goes to the detective agency and meets Kogoro. Sonoko (now 27 years old) drops in and tells him Araide proposed to Ran, but Ran was still waiting for Shinichi.
Conan tries to search for Ran, he searches for her in Tropical Land, and there we see a lot of flashbacks from Shinichi and Ran being together there and when Shinichi was attacked.
He finally finds Ran in his library. I think he asks her to keep waiting for Shinichi. Ran tells him she rejected Araide. Then Conan finally tells her that he's really Shinichi. But Ran thinks he only says that to make her stop worry. Then he start turning back to the 6 year old Conan again.
Conan wakes up. He's been fever dreaming (or whatever it's called). Ai says he was uncontious and in pain since he transformed into Shinichi.
The detective boys eats dinner at the detective agency. And Araide, making Conan very jelous 8D

I don't understand Japanese, so this is just what I understood (except from the Conan/Ran scene which some people translated for me =D ).

Though the whole OVA was not "realistic" in the DC world it was fun to watch. Realy. I enjoyed it. I liked the ending credits as well. X3

It´s really bothering me how everyone claims that Shinichi is the egoist while Ran is the "poor victim". Even though he said that it would be okay if she forgets about him and just get happy, he still loves her and I think it´s pretty understandable that he wouldn´t all cheery about Ran moving on while he himself has to live as another person.
As you said, it´s a nightmare. He knows and thinks that if there´s really no way out, Ran should forget about him- but can you blame him for fearing that situation? If he had no problem with this thought at all, someone should give him the prize for being the most selfless person in the world- and that would give him more Mary-Sue-characteristics which you all seem to hate on him.

Someone who cares about her? That Shinichi cares so much about Ran is actually the reason why they got in this situation in the first place.
Sexism much? Just because he´s guy doesn´t mean that he isn´t suffering just as much as Ran- or a lot more.
Shinichi prooved many times that he cares more about Ran´s feelings and well-being than about his own. Does the fear that Ran might forget about him immediately makes him an egoist?
