Well I think that Conan just is dreaming..
Of course after 10 years he think that Ran will be with Eisuke, because he already knows that Eisuke loves ran.

It doesn't have to be a dream necessarily, it's and OVA and it is non-cannon so I don't see why it couldn't just be a silly way to see the show, all four movies of Dragon Ball use the Original Characters, but in totally diferent scenarios and there was no explanation given, beacuse they really don't need to explain why something happens if is not related to the story

The description of the OVA said that Conan fell uncontionus because of a cold, and when he woke up, the world seemed to have changed. And with the pictures, it easy to think that's it's all a dream.
