interestingly how did they choose? it's not like these two are the same age, so there must be a point where the BO went "Ok, pick this one".... plus, there was a part where Akemi came with her parents, spoke with them in Japanese so I assumed she was about 5-6, but Ai wasn't there. so did BO chose Ai based on her brain or something else?
Possible they did something to make sure that their first daughter wasn't used by the BO, but when they died shortly after their second's birth, they didn't get the choice.

I don't think it's known at the moment if they were separated or if they chose individual lives.
It's highly likely that the Miyanos were extremely respected individuals in the Organization and upon their death, their children received special treatment from "The Boss". This meant the best education, lifestyle, and career possible all for their choosing.
So it could be possible that Akemi just refused all of this as she was much older than Shiho at the time and ultimately refused the Organization while Shiho embraced all of it trying to carry on her parent's research as a means to feel close to them.
Yeah, these are things Gosho needs to address instead of creating cases that have nothing to do with the plot.
