Actually, I've seen a bunch of guys with shaved heads who have a bald spot like this. Of course, when under pressure to come up with a picture or an example, I can't I think maybe it's to represent a shaving knick or something (from a homebrew haircut).

I think we all have small spots on our heads like this, probably not as big as Genta's (his I think is enlarged for the sake of him being a caricature) but we don't notice them, because our hair is so long it covers it up. My ex actually had a bald spot about the size of a dime on his head, and of course he routinely kept his hair so short it was pretty noticeable...
