My thoughts:
I think the Mystery Lady is more likely to be Yukiko than Fusae.

We don't know anything about Fusae, what her abilities are and what use she can be in the case. But we do know that Yukiko is able to disguise herself and others.
Also, that Mystery Lady confronted Scar Akai who's likely Vermouth. So, if Fusae is an undercover agent, why would she reveal herself to Vermouth? To scare away Vermouth?
That way, Vermouth would be warned that her cover is blown.

If it would be Yukiko tho, then Okiya/Akai can use her ability for disguises to save Shiho. Okiya likely send that message to Haibara to force her to take the APTX (like already said). Once she's big again, they can disguise her. First I thought she might be disguised as Ran or Sonoko (since sonoko was compared to Haibara because of the hair once XD). But since a uniform is missing, they could disguise her as the staff, since the BO is looking for Sherry as a passenger on that train.
Also, Yukiko was friends with Vermouth, so her encountering Vermouth with a "Hi Sharon" etc. would shock Vermouth more. And would pressure Vermouth more too and wouldn't immediately make Vermouth think that she's spotted by the FBI.
Akai also had a photo where the DBs and Yukiko are on it. Jodie/Akai were also there when Conan disguised himself as Haibara. Okiya also lives on Shinichis house, and Yukiko already made a surprise visit once to scare conan too. So it's not impossible for Okiya/Akai to get to know Yukiko and her skills.

Random twist: Yusaku is disguised as Kogoro! *_* XD

Oh yea, anyone else thinks that Ran will see Haibara turning into an adult again? Thus blowing Conans cover? XD

I was thinking about this, well not this specifically, but something similar.  If they really don't want to stop at Nagoya (assuming they know the BO are waiting)... why not just call in a bomb threat?  At the very least, it'd force the train to either detour or stop one stop sooner.  All they'd need is a way to make the call untraceable, and with the FBI around that should be easy.

Okiya never said that the murder was a chance. He said heaven/fortune was siding with them which is much more vague. Maybe he is happy Amuro hasn't noticed Haibara yet?

So that's why I feel weird about the door because of that trick, you know, why would Andou-san close the door from outside like pushing it, thought it was hiding, not from inside and by knob, you know, like normal closing the door when go out.

Great job! I didn't even notice the mirror outline until you pointed it out. I only have one question:

I'm still not clear on how they got the gun into the victim's hand to look like a suicide. Can someone explain that part to me? They shot him from outside to make it look like a locked room suicide, right? So how did the gun end up in his hand? You probably explained it, but I missed it.

The other is they asked him to shoot the couch in front of him with it. The victim set up the locked room by himself, following directions from the culprits without knowing they were going to use his actions to murder him.

If the door we see is mirrored, then it must be the "outside", so we should see the e-tag on the door. But that would have been too obvious I guess, so Gosho didn't draw it.

I'm sure Ran has got a big reason to be there apart from protecting Ai or resembling Akemi: She knows Akai and thus can possibly blow Scar Akai's cover.

No-one except the real Akai, Ran and Sonoko know that they met so Vermouth (or someone else) can't have prepared for that scenario. Akai left quite the impression on Ran so it's unlikely that she forgot about him. She could realize that his attitude is different, say something about 'always crying whenever they meet', ask about his scar, ...

I've been hoping she'll have a really big role this time ever since her face appeared on that trailer-picture (Kogorou, Sonoko and the DB aren't shown) and with her being outside of the save cabin her potential increased a lot.

And on a side note: If Yukiko is the mysterious woman and has been living with Okiya in the Kudo manor and helping with his disguise: why did Sera find a long hair (!) from a wig? It can't be Okiya's (short hair) and Yukiko wouldn't have a reason to wear a wig inside her own house.

Sani's point isn't so much about Ran being able to notice a mask, I don't think, and more about how the BO doesn't know that Ran and Akai know each other, so if Ran recognizes Scar Akai as Akai, and Scar Akai doesn't recognize her back, she might realize something's up.

Although, I sorta doubt that's gonna be the case, I feel like it's more probable she'll have a bigger role in this case from interacting with Haibara than Akai/Scar Akai.

3. Is it just me or is Sonoko's manner a little bratty in these chapters?

1. I noticed that and thought it could indicate that his bell had been disabled so that only the culprit could ring it and control the movement of the attendant, but it is possible Room A guy was referring to when he originally tried to call the attendant before the attendant realized the light was out. I couldn't rule out the second idea although it was suspicious.

2. I thought it was the lady who had a two part bubble, not the conductor speaking. That little pointer makes it vague who is speaking...

3. She seems a bit shorter-tempered than normal, especially at Conan for solving the mystery but it isn't at a level which is out of character for her.
