Well, that is actually a good question.

I think first she will be very confused. And then... well, I guess it depends on who is supporting her in that situation and if Conan got an Antidote to become a teenager again. If he does she will have no choice but to give up on him - even if it takes her a long time to do so. She might remain having a lingering crush on him, but no real chance.
But if he is still small at that time - why should she give up? On the contrary, knowing his secret might allow her to understand Conan/Shinichi a bit better to allow her getting even closer to him. I guess it would all depend on realizing that knowing about Conan's "past" does not change who he really is.

So: I think she will still remain in love with Conan, even if she knew is true identity.

Sure? Shinichi loved the attention of other girls, even though he was not really interested in them because he had a special girl in his mind all the time.
So now he has a little girl falling in love with his younger self - why should he not think it is cute?
Of course... he is not interested in her (for obvious reasons). But I think he is still flattered by her attention.

This is what I've been thinking about for a number of weeks when I am idling and not doing anything. I have a few scenarios, but they're long and complicated, and I don't feel like writing them down.

B:  It gets the other Detective Boys pissed at him and he doesn't need those headaches.
