Umm, no. Whether the producer's hand is injured or not has no merit at all. It ended because the mangaka has been hospitalized multiple times over the last few years, and the anime was about to catch up to the manga.
It very much is the last episode. Whether there's a continuation later or not, this is the last episode as of now.

Wrong again. Shows don't start in November, and it would've been announced by now, which it hasn't been.

Hasn't been directly stated yet, though Dorian Gray is the likely allusion. Whether Dorian Gray is supposed to be represented by the Earl, the 14th, Allen, Kanda, whoever, is unsure.

Just a nifty tidbit: The pilot chapter for the series was called "Zone", and this series was originally going to be called "Dolls."
