Actually it's a seinen published in a seinen magazine

Alicetama is correct. It's one of the latest chapters of Hourou Musuko (Wandering son).A more or less realistic story about a preadolesecent boy who wants to be a girl who changes schools and ends up into a class with a girl who wants to be a boy. They made an anime of it recently but the anime was quite sucky because they couldn't put the truckload of flashbacksthe manga has and they minimize the transexual issue.
